Why build Omnisend forms in ConvertFlow
Omnisend's built-in signup forms do one job well: capture an email or phone number in exchange for a discount. That works until you need the form to do more than collect an address.
ConvertFlow forms sync into the same Omnisend contact record, but add multi-step funnels that qualify before asking for contact info, conditional paths that show different offers to different segments, custom fields written on every answer, Shopify product recommendations with add-to-cart, and A/B testing with revenue attribution.
The contacts still live in Omnisend. ConvertFlow is the capture layer in front of it.
1. Connect Omnisend to ConvertFlow
In Omnisend, go to Account → Settings → API Keys and create a private API key for ConvertFlow.

Paste it into the Omnisend integration form in ConvertFlow's Integrations area. Once connected, ConvertFlow pulls in your Omnisend campaigns and tags, so you can trigger automations and segment subscribers as shoppers submit your funnels.

2. Add a form to your campaign
Add your Omnisend form to a ConvertFlow landing page, popup or embed. Click to add an element and choose the "form" element type — you can start from a form template or build from scratch.

3. Toggle the contact fields you want
Select the form element and switch on the contact fields to request. ConvertFlow's preset fields are already configured to send to your integration's contact fields — email address and first name map to Omnisend automatically.

Each field can be edited from the builder panel — placeholder text, label, required state and more.

4. Verify emails and phone numbers
Optionally have ConvertFlow check that an entered address exists with its provider before it reaches Omnisend, which protects your list quality and sender score. Phone verification formats numbers to the E.164 standard that SMS tools require.

5. Add custom fields
Add as many custom fields as you need. Use the "New field name +" option to name one — Omnisend creates the matching custom field on their side automatically the first time a contact is sent, so nothing needs configuring in Omnisend first.

Choose the field type — text, dropdown, radio, paragraph, range or hidden. Dropdown and radio options can carry score values for funnel logic.

6. Map survey and quiz answers to Omnisend
Survey elements have a "Map to custom field" dropdown. Each answer is written to the subscriber's Omnisend record — which is what makes answer-based segmentation possible.

7. Map ConvertFlow's preset fields to Omnisend
With Omnisend connected, your Integrations page shows a Fields button. Use it to map preset contact fields to Omnisend custom fields by entering the Omnisend data name. Email address and first name are already mapped — use this for the rest, like last name, phone and company.

8. Style the form to match your brand
In the Design tab, set spacing, field layout, width, fonts, button colour and roundness, field colour and shadows so the form looks native to your site rather than bolted on.

9. Configure what happens on submit
In the Actions tab, choose what follows submission — redirect to a URL, jump to another funnel step, display a thank-you message, or open a popup.

Alongside the default action, add automations that fire on submit — including which Omnisend list or tag the contact should be added to, so segmentation happens at the moment of capture rather than in a later sync.
Conditional actions let you do different things for different people — routing by field value, so a high-intent answer goes somewhere different from a browser.

10. Report on submissions
The funnel breakdown report shows how many people saw, converted and completed the form, where they dropped off, and which answers they selected — with orders, revenue and AOV attributed to each answer.

11. Segment and personalize in Omnisend
Once answers are landing on Omnisend contact records, build Omnisend segments from those field values, and personalize your website for Omnisend subscriber segments — showing returning subscribers a different offer instead of the same signup form.
12. Publish
- On Shopify: publish to a Shopify page in one click.
- On WordPress: add ConvertFlow's snippet once, then target the form to specific posts, pages or categories.
- Anywhere else: Webflow, Squarespace, Wix or any CMS take the same single embed snippet.
The same form works as an embed, popup, slide-up, full-screen section or hosted landing page without rebuilding it.
