Why build Klaviyo forms in ConvertFlow
Klaviyo's built-in signup forms do one job well: capture an email or phone number in exchange for a discount. That works until you need the form to do more than collect an address.
ConvertFlow forms sync into the same Klaviyo profile, but add multi-step funnels that qualify before asking for contact info, conditional paths that show different offers to different segments, profile properties written on every answer, Shopify product recommendations with add-to-cart, and A/B testing with revenue attribution.
The profiles still live in Klaviyo. ConvertFlow is the capture layer in front of it.
1. Connect Klaviyo to ConvertFlow
Head to your ConvertFlow website's Integrations area and click to connect with Klaviyo.

There's no API key to generate. Klaviyo connects over OAuth — you'll be asked to give ConvertFlow permission to access the data and API endpoints it needs. Review those, then click Allow.

Klaviyo and ConvertFlow are now connected. ConvertFlow starts pulling in your Klaviyo lists and segments, so you can trigger flows and segment subscribers as shoppers submit your forms, landing pages and quizzes.
2. Keep Klaviyo's site tracking working
This step is specific to Klaviyo and easy to miss. For Klaviyo's site tracking and JavaScript events to work alongside ConvertFlow's forms, Klaviyo's tracking script needs to be installed on the same page as ConvertFlow.
With both on the page, ConvertFlow automatically uses Klaviyo's tracking API to identify shoppers who submit their email — so browsing behaviour stays attached to the profile, and your browse-abandonment flows keep working exactly as they would with a native Klaviyo form.
3. Add a form to your campaign
Add your Klaviyo form to a ConvertFlow landing page, popup or embed. Click to add an element and choose the "form" element type — start from a form template or build from scratch.

4. Toggle the contact fields you want
Select the form element and switch on the contact fields to request. ConvertFlow's preset fields are already configured to send to your integration's contact fields — email address and first name map to Klaviyo automatically.

Each field can be edited from the builder panel — placeholder text, label, required state and more.

5. Verify emails and phone numbers
ConvertFlow can check that an entered address exists with its provider before the profile reaches Klaviyo, which protects your list quality and sender score. Phone verification formats numbers to the E.164 standard that Klaviyo SMS requires — worth enabling if you're collecting phone numbers for SMS consent.

6. Add consent checkboxes
Scroll below the preset contact fields to find compliance checkboxes:
- Privacy consent: link to your privacy policy and terms of service alongside a required checkbox.
- Marketing consent: if your form gates content but also opts the shopper into ongoing marketing, collect consent for it.
- SMS consent: require consent to SMS marketing with the required disclosures.
Consent values can be sent to the profile in Klaviyo by mapping these preset fields in your integration settings.
7. Add custom fields — Klaviyo creates them for you
By clicking to add a field at the end of the fields list, you can add as many custom fields as you'd like.

Use the "+ New field name" option to name one. Klaviyo creates the matching profile property automatically the first time a profile is sent, so there's nothing to configure in Klaviyo first — you can design the form around the data you want and let the properties appear as they're used.

Choose the field type — text, dropdown, radio selection, paragraph or hidden. Dropdown and radio options can carry score values for funnel logic.

8. Map survey and quiz answers to Klaviyo
Survey and quiz elements have a "Map to custom field" dropdown. Each answer is written to the shopper's Klaviyo profile — which is what makes answer-based segmentation possible, and what turns a quiz into a segmentation tool rather than just a lead magnet.
9. Map ConvertFlow's preset fields to Klaviyo
With Klaviyo connected, your Integrations page shows a Fields button. Use it to map ConvertFlow's preset contact fields to Klaviyo properties by entering the property's data name. Email address and first name are already mapped — use this for the rest, like last name, phone and company.

10. Resync when you add something new in Klaviyo
If you've just created a new list or segment in Klaviyo and it isn't appearing in ConvertFlow's dropdowns, refresh the integration to pull the latest account data.
11. Style the form to match your brand
In the Design tab, set spacing, field layout, width, fonts, button colour and roundness, field colour and shadows — so the form looks native to your store rather than bolted on.

12. Configure what happens on submit
In the Actions tab, choose what follows submission — redirect to a URL, jump to another funnel step, display a thank-you message, or open a popup.

Alongside the default action, add automations that fire on submit — including which Klaviyo list the profile is added to, so segmentation happens at the moment of capture rather than in a later sync.
Conditional actions let you do different things for different shoppers — routing by field value, so a high-intent answer enters a different flow than a browser.

13. Report on submissions
The funnel breakdown report shows how many people saw, converted and completed the form, where they dropped off, and which answers they selected — with orders, revenue and AOV attributed to each answer.

14. Segment and personalize in Klaviyo
Once answers are landing on Klaviyo profiles, build Klaviyo segments from those property values and target your flows against them.
ConvertFlow can also read your Klaviyo segments and personalize your store for them — showing returning subscribers a different offer instead of the same signup form.
15. Publish
- On Shopify: publish to a Shopify page in one click.
- On WordPress: add ConvertFlow's snippet once, then target the form to specific posts, pages or categories.
- Anywhere else: Webflow, Squarespace, Wix or any CMS take the same single embed snippet.
The same form works as an embed, popup, slide-up, full-screen section or hosted landing page without rebuilding it.
