Playbook summary

Sell a personalized weight loss plan and supplement bundle, not a single product. A shopper answers a short quiz about their goal, health, and habits, gives their email and phone for their plan, sees a recap of their goal, pace, and motivation, then lands on one results page with their forecast, a plan + supplement hero, a free-gift bar, three add-ons, and FAQs.

  • ESP and SMS: Klaviyo, Attentive, Postscript, or equivalent (not in the template; connect it on the email and SMS steps)

Quick setup

  • Content: 12 questions (goal, health profile, habits, pace, motivation) plus a loading screen inside one quiz element
  • Call-to-action: Get the plan by email and text, then start the plan + supplement bundle and add the routine in one click

Use this template to sell a personalized weight loss plan and supplement bundle, not a single product. A shopper answers a short quiz about their goal, health, and habits, gives their email (and optionally SMS) to get their plan, sees a recap of their goal, pace, and motivation, then lands on one results page with their forecast, a plan + supplement hero, a free-gift progress bar, three add-ons they can add in one click, and FAQs.

That path is a personalized shopping funnel: the shopper builds a plan, sees what it looks like for them, and buys it, with email and SMS follow-ups personalized from their quiz answers.

Weight loss is where quiz funnels earn their keep. Programs like Noom and telehealth brands like Hims & Hers open with a quiz because shoppers can’t tell which plan, pace, or product fits them, and a generic product page doesn’t answer that. When someone takes a quiz, they self-identify their goal, constraints, and motivation, so you can recommend a plan built for them instead of asking them to assemble one from a catalog.

An email gate sits before the results, so you keep the subscriber even when they don’t check out.

Performance benchmark

A typical Shopify store converts 1.4–2.6% of sessions into orders (based on Littledata’s and Shogun’s benchmarks).

Based on ConvertFlow’s September 2026 Product Quiz Funnel Benchmark, sampled across 2,120 ConvertFlow product quiz funnel campaigns, active Shopify product quiz funnels engage 44.2% of visitors, turn 5.2% of quiz viewers into orders, and earn $9.43 in attributed revenue per view on average. Orders placed through the quiz average $264.

Basket size matters. Directionally, stores with a $150+ AOV see a median 60.6% quiz engagement and $13.49 per view, while stores under $50 see 15.6% and $0.24. A plan plus supplements is a higher-consideration purchase than a single SKU, which is where a guided quiz helps most. Measure your funnel against your own AOV band.

ConvertFlow customers see it in their own numbers:

Core principles

Sell the plan, not the product: The quiz exists to end choice overload. The hero is the plan or lead bundle; the three add-ons around it complete the routine. If every answer combination lands on the same SKUs, you haven’t built a plan, you’ve built a catalog.

Ask health questions only when they change the answer: This template asks about pregnancy, conditions, and diabetes because a responsible weight loss plan can’t ignore them. Every one of those questions should change what the shopper sees, either the plan you recommend or whether you recommend one at all. If an answer doesn’t change the result, cut the question.

Don’t split data collection from the sale: The same path that recommends the plan must write answers into your ESP as custom fields or profile properties, add the subscriber to a list, and fire a quiz-completed event. If the ESP isn’t connected, the funnel isn’t live.

Eliminate dead ends: After the results, the shopper starts the plan or adds to cart. The hero, the free-gift bar, and the three add-ons are the offer. A thank-you page with no product is a failed activation.

Execution strategy

Lead with the outcome: Step 1 is a landing intro: “Lose the weight. Keep the life. Take the 3-minute quiz to build a plan around your habits, not someone else’s. Your coach adjusts it as you go.” Button: Start my plan. Ask about their goal first, not about your ingredients.

The funnel is 6 campaign steps:

  1. Intro (one step): headline, subhead, Start my plan button, brand image
  2. Quiz (one step, 12 questions inside one quiz element, plus the loading screen): goal, gender, pregnancy, age, height, weight, conditions, diabetes, focus area, upcoming event, pace, motivation. After the last question, the quiz shows “Building your custom plan…” then advances
  3. Email gate: “Where should we send your plan? We’ll email you a summary of your custom plan plus your trial link.” First name, email, consent. Button: Show my plan
  4. SMS: “Get your plan by text.” Phone with country code and SMS consent. Button: Subscribe For Texts. Link: No Thanks
  5. Plan ready (one step): “Your plan is ready!” Personalized line, recap of goal, pace, and motivation. Button: See My Bundle
  6. Results (one step): forecast, plan + supplement hero, trust ticks, free-gift bar, three add-ons with Add All To Cart, FAQs

Gate the plan, then make the offer: Email and SMS come after the quiz and before the plan. Benefit copy on the buttons (“Show my plan”), not “Submit.” The free-gift bar on results is the incentive to add the routine.

Use quiz answers for follow-up: Send answers into your ESP so the welcome flow leads with what they told you. A shopper with a wedding coming up and a “Look better in clothes” motivator hears about their plan and their date, not a generic welcome.

Pro tips

Keep the CTAs benefit-driven. Template buttons are “Start my plan”, “Show my plan”, “See My Bundle”, and “Start Now”. Do not change them to “Submit” or “Sign up”.

Route shoppers who shouldn’t get a plan. A shopper who answers “Yes” to pregnancy, or has type 1 diabetes, shouldn’t land on a weight loss forecast. Send those answers to a results variant that recommends talking to their doctor, or to a provider consult if you offer one.

Keep health answers out of ad pixels. Weight, pregnancy, conditions, and diabetes answers are consumer health data under state privacy laws such as Washington’s My Health My Data Act. Send them to your ESP, not to Meta or Google. If you pass a conversion event to ad platforms, pass the event only, without the answer values. If you’re a telehealth provider covered by HIPAA, every tool that receives these answers needs a Business Associate Agreement.

Keep the forecast honest. The results page shows a projected loss over 6 months. Make sure the math is a healthy, realistic pace, label it as an estimate, and pair it with “results vary” language. Specific weight loss promises need substantiation under FTC rules.

Launch as a dedicated landing-page URL so ads, nav, welcome popups, and your ESP can all point at one quiz. Popups and embeds come after this URL works.

Do not invent extra steps. The loading screen lives inside the quiz element. The plan-ready step is not optional filler. Results is one scroll, not “results” plus “upsell” plus “FAQ” as separate steps.

Quick setup

1. Start with the Weight Loss Product Bundle Quiz template

Do not build from a blank canvas. Use our quiz funnel template for this.

Open the template, click Use This Template, keep the 6-step structure. Use AI to remix the logo, colors, the images, and copy. The campaign is a landing page. It is not a popup and it is not one step per question.

Step 1 of the Weight Loss Product Bundle Quiz Funnel: Lose the weight. Keep the life. intro with Start my plan button

What you should see in the builder: 6 steps. Step 2 holds one quiz element with 12 questions and a loading screen. If you see yourself adding steps for each question, stop.

2. Set up the quiz questions

The 12 questions in this template, in order, all live in one quiz element. Single-select questions auto-advance; multi-select and number questions show Continue. Suggested custom fields: weight_goal, gender, pregnant, age_range, height, current_weight, conditions, diabetes, focus_area, upcoming_event, pace, motivator.

Quiz loading screen: Building your custom plan, analyzing your goals, calculating your healthy pace, finalizing your forecast

Loading screen after question 12: “Building your custom plan… Analyzing your goals. Calculating your healthy pace. Finalizing your forecast.” Keep it. It sells the plan.

Swap the labels for your program, but keep question 1 about the shopper’s goal and keep the health questions you actually use to route the plan. Height and current weight feed the forecast on the results page. Map each question to its custom field so answers land in ConvertFlow and in your ESP.

Set readable answer values: double-click an answer to open its settings. Each answer has a label, which the shopper sees, and a value, which is stored in the custom field and shown in merge tags and the recap on step 5. Change values to the words you want in the recap, your ESP segments, and your product filters.

3. Connect your ESP on the email step

Step 3 email gate: Where should we send your plan? First name, email, consent, Show my plan button

Go to step 3 (the email gate). Select the form. Actions: connect Klaviyo or your ESP if it isn’t already. Then add automations:

  • Add to list or audience (your quiz or main list)
  • Write quiz answers to custom fields or profile properties
  • Track a quiz-completed event

In the builder: select the form, open the Actions tab, click to add or manage integrations, find your ESP, and click Connect. Then scroll to Automations and click Add Automation, for example “Klaviyo – Add to list”.

Adding a Klaviyo add-to-list automation to a ConvertFlow form

Form in the template: first name, email, email consent, Accept privacy policy and terms of service. Headline: Where should we send your plan? Subhead: We’ll email you a summary of your custom plan plus your trial link. Button: Show my plan.

If this step is skipped, the trial is not activated. Activation is ESP connected plus at least one billable action (list, field, or event). Preview will not fire automations. That is expected. The connection still has to exist.

4. Connect SMS on the SMS step

Step 4 SMS: Get your plan by text, phone number with country code, SMS consent, Subscribe For Texts and No Thanks

Step 4 asks for a phone number with country code and an SMS consent checkbox. Headline: Get your plan by text. Button: Subscribe For Texts. Link: No Thanks. Add an automation to subscribe the phone to your SMS list (Klaviyo SMS, Attentive, Postscript, or equivalent), and update the consent text with your brand name.

5. Set the plan-ready step

Step 5: Your plan is ready! with a recap of your goal, your pace and your motivation, and a See My Bundle button

Step 5 in this template is one screen:

  • Headline: Your plan is ready!
  • Personalized line: “{first name}, based on your answers, we’ve mapped out a personalized path to your goal weight.” Set a fallback first name (for example “friend”)
  • Recap: Your goal, Your pace, Your motivation
  • Button: See My Bundle

Only 3 of the 12 answers recap. Keep the health answers off this screen. The recap reads the stored answer values, so fix them on the answers in step 2 rather than here.

6. Connect Shopify and build the results page

Step 6 results: Your forecast, you can reach your goal, plan plus daily supplement bundle hero with Start Now, trust ticks, free gift progress bar, add-ons with Add All To Cart, and FAQs

Connect Shopify under Integrations, then return to step 6. The results page is one scroll:

  • Eyebrow: YOUR FORECAST. Headline: You can reach your goal. Subhead: Based on your answers, here’s a healthy weight loss trajectory for you
  • Forecast: “__ lbs less in 6 months,” filled from the shopper’s answers (it shows blank in the template preview)
  • Hero: the product hero with your plan + supplement bundle (demo: TrimPath Plan + Daily Supplement Bundle) and a Start Now button. Use conditional recommendations so each goal and focus area gets its plan
  • Trust ticks: Personalized for you / 12,000+ happy customers / Trusted by influencers (swap the number for a real one)
  • Free gift: “Upgrade your plan and get a free gift.” Progress bar with the amount left and a Checkout button. Set your threshold
  • Add-ons: the products feed capped at 3, each with a 25% OFF badge and Add To Cart, plus Add All To Cart
  • FAQs: How is my plan personalized for me? / What’s your return policy? / Can I retake the quiz for a new plan? (rewrite the answers for your policies)

How the product settings work

Choose the product source: select the product hero or products feed, then pick the source. Use your connected Shopify store, or ConvertFlow custom products for a plan or bundle SKU you’ve set up separately.

Choosing the product source and filters for a ConvertFlow products element

Add conditional recommendations per answer: click to add a conditional product recommendation, add conditions based on quiz answers (ALL or ANY must match), then set which products show for that condition. Do this on the hero and on the add-ons feed so both change together. No separate campaigns or steps are needed for each plan.

Or let the quiz pick the products: the Quiz Auto Recommendation filter uses submitted answers and custom fields to filter your catalog automatically. Exclude questions that shouldn’t affect the products, like gender or upcoming event.

Cap the add-ons at 3: in the products feed’s design settings, set the maximum number of products to 3.

Products feed content and design settings in ConvertFlow

Don’t split the forecast, hero, gift bar, add-ons, and FAQs into separate steps. Keep it as one personalized sales page (you’ll link back to it from email and SMS).

7. Launch the URL, then preview the full path

Set the landing page domain and path. Save. Preview the whole funnel: intro, questions 1 through 12, loading, email, SMS, plan ready, results.

Launching a ConvertFlow quiz funnel as a landing page with a custom domain and path

Confirm:

  • 6 campaign steps
  • First name merges into the plan-ready line, with a fallback when empty
  • Recap shows readable values for goal, pace, and motivation, and no health answers
  • The forecast fills in with a realistic number
  • Pregnancy and type 1 diabetes answers route away from the forecast
  • Forms submit (automations won’t run in preview), and No Thanks skips SMS
  • Real products show in the hero and add-ons, not placeholders
  • Different goals produce different plans
  • Start Now, Add All To Cart, and Checkout all work
  • No ad pixel receives quiz answers
  • No thank-you dead end

Then activate. In the dashboard, watch completion, answer mix, attributed orders, and AOV against the benchmark for your AOV band.

In the dashboard: you can see how many visitors progress through each step, which answers they pick, and the orders, revenue, and AOV attributed to each answer.

Then split-test: once you have orders, test question count (12 vs. fewer), the hero offer (plan + supplement vs. supplement only), and the free-gift threshold.

Follow up with personalized email and SMS flows

Most quiz takers won’t check out on their first visit. Their goal, pace, and motivation are the best follow-up you have.

8. Build a quiz follow-up flow in your ESP

Trigger it off the quiz: start the flow from the quiz-completed event or the list from step 3. Exit anyone who orders.

Split by answers, not by health data: use conditional splits on goal, focus area, pace, motivation, and upcoming event. Keep health answers out of subject lines and SMS text, since messages show up on lock screens.

  • Email right away: their plan and the link back to their results
  • Email a day later, if they haven’t ordered: how the plan works for their focus area, plus reviews
  • SMS for shoppers who opted in: a short reminder with the same link
  • If they named an event, a countdown email tied to it

9. Skip returning visitors straight to their results

Without skip logic, someone who clicks back from email lands on the intro and has to retake the quiz. Skip logic checks each visitor when they reach a funnel step and jumps them to the right one.

Adding skip logic to a funnel step in ConvertFlow

In the builder: click to manage your funnel, edit the intro step, and click + Add Skip Logic. Add the condition that the visitor has already completed the quiz, then under Jump To A Funnel Step choose the results step. Add the same logic to the email and SMS steps so a known subscriber never sees the form twice.

Skip logic settings: if conditions are met, jump to the results funnel step
