Use this template to sell a personalized hair regrowth routine, not a single product. A shopper answers five quick questions about their hair loss and goals, gives their email (and optionally SMS) to get their plan, sees a recap of their goal, where they’re noticing changes, and the results they want, then lands on one results page with a treatment hero, a free-gift progress bar, three add-ons they can add in one click, and FAQs.

That path is a personalized shopping funnel: the shopper describes their hair loss, sees the routine built for it, and starts it, with email and SMS follow-ups personalized from their quiz answers.

Hair loss brands like Hims and Keeps open with a quiz for a reason. Shoppers can’t tell whether they need a topical, an oral, a supplement, or all three, and a catalog of serums and shampoos doesn’t answer that. When someone takes a quiz, they self-identify their pattern, timeline, and goal, so you can recommend a routine built for them.

An email gate sits before the results, so you keep the subscriber even when they don’t check out.

Performance benchmark

A typical Shopify store converts 1.4–2.6% of sessions into orders (based on Littledata’s and Shogun’s benchmarks).

Based on ConvertFlow’s September 2026 Product Quiz Funnel Benchmark, sampled across 2,120 ConvertFlow product quiz funnel campaigns, active Shopify product quiz funnels engage 44.2% of visitors, turn 5.2% of quiz viewers into orders, and earn $9.43 in attributed revenue per view on average. Orders placed through the quiz average $264.

Basket size matters. Directionally, stores with a $150+ AOV see a median 60.6% quiz engagement and $13.49 per view, while stores under $50 see 15.6% and $0.24. Hair regrowth is a months-long routine, so the value of each quiz order shows up in subscriptions and repeat orders, not just the first cart. Measure your funnel against your own AOV band.

ConvertFlow customers see it in their own numbers:

Core principles

Sell the routine, not the product: The hero is the lead treatment; the three add-ons around it complete the routine (a supplement, a shampoo, a serum). If every answer combination lands on the same SKUs, you haven’t built a routine.

Ask what they see: Every question in this template is about what the shopper notices (where, how much, since when, what they want). That’s enough to match a routine and it keeps the quiz to five questions.

Don’t split data collection from the sale: The same path that recommends the routine must write answers into your ESP as custom fields or profile properties, add the subscriber to a list, and fire a quiz-completed event. If the ESP isn’t connected, the funnel isn’t live.

Eliminate dead ends: After the results, the shopper starts treatment or adds to cart. A thank-you page with no product is a failed activation.

Execution strategy

Lead with the outcome: Step 1 is a landing intro: “Treatment that works as hard as you do. A 2-minute quiz. A licensed provider. A regimen designed for your scalp, delivered to your door, adjusted as you progress.” Button: Take the free quiz.

The funnel is 6 campaign steps:

  1. Intro (one step): headline, subhead, Take the free quiz button, brand image
  2. Quiz (one step, 5 questions inside one quiz element): hair loss stage, area, amount, onset, expected results
  3. Email gate: “Almost there — where should we send your plan? A licensed provider will review your answers and recommend a personalized treatment.” First name, email, consent. Button: See My Results
  4. SMS: “Get SMS updates. Want your treatment plan sent to your phone?” Phone with country code and SMS consent. Button: Subscribe For Texts. Link: No Thanks
  5. Results ready (one step): “Your results are ready!” Personalized line, recap of hair goal, where they’re noticing changes, and expected results. Button: See My Results
  6. Results (one step): treatment hero with a purchase-frequency selector, trust ticks, free-gift bar, three add-ons with Add All To Cart, FAQs

Gate the plan, then make the offer: Email and SMS come after the quiz and before the plan. Benefit copy on the buttons (“See My Results”), not “Submit.”

Use quiz answers for follow-up: Send answers into your ESP so the welcome flow leads with their pattern. A shopper with a receding hairline who wants “a stronger, defined hairline” hears about hairline treatment, not a generic welcome.

Pro tips

Keep the CTAs benefit-driven. Template buttons are “Take the free quiz”, “See My Results”, and “Start Now!”. Do not change them to “Submit” or “Sign up”.

Default to a subscription. Hair regrowth takes months to show, so set the hero’s frequency selector to your subscription and make one-time purchase the alternative. A one-month order often gets returned before the treatment has had time to work.

Only claim a licensed provider if you have one. The template copy says a licensed provider reviews answers, and the results page shows “Doctor reviewed”. Keep that language only if a provider actually reviews them. For prescription treatments, the quiz is the start of a medical intake and needs a real provider consult behind it.

Match claims to the product. FDA-approved OTC treatments like minoxidil can make the regrowth claims on their label. Supplements and cosmetics can’t claim to treat or regrow hair loss; use language like “supports thicker, fuller-looking hair.”

Keep answers out of ad pixels. Hair loss answers can count as consumer health data under state privacy laws such as Washington’s My Health My Data Act. Send them to your ESP, not to Meta or Google. If you pass a conversion event to ad platforms, pass the event only.

Launch as a dedicated landing-page URL so ads, nav, welcome popups, and your ESP can all point at one quiz. Popups and embeds come after this URL works.

Do not invent extra steps. Results is one scroll, not “results” plus “upsell” plus “FAQ” as separate steps.

Quick setup

1. Start with the Hair Growth Product Bundle Quiz template

Do not build from a blank canvas. Use our quiz funnel template for this.

Open the template, click Use This Template, keep the 6-step structure. Use AI to remix the logo, colors, the images, and copy. The campaign is a landing page. It is not a popup and it is not one step per question.

Step 1 of the Hair Growth Product Bundle Quiz Funnel: Treatment that works as hard as you do intro with Take the free quiz button

What you should see in the builder: 6 steps. Step 2 holds one quiz element with 5 questions. If you see yourself adding steps for each question, stop.

2. Set up the quiz questions

The 5 questions in this template, in order, all live in one quiz element. Single-select questions auto-advance; the multi-select question shows Continue. Suggested custom fields: hair_goal, hair_loss_area, hair_loss_amount, hair_loss_onset, expected_results.

Hair growth quiz question 1: Which best represents your hair loss and goals? Receding hairline, want to slow its progress, Experiencing hair loss, exploring options, Experiencing hair loss, ready to start treatment ASAP, No hair loss yet, want to get ahead of it, None of the above

1. Which best represents your hair loss and goals? Single-select: Receding hairline, want to slow its progress / Experiencing hair loss, exploring options / Experiencing hair loss, ready to start treatment ASAP / No hair loss yet, want to get ahead of it / None of the above

Hair growth quiz question 2: Where are you noticing changes to your hair? Along the hairline, At the top, All over

2. Where are you noticing changes to your hair? Single-select: Along the hairline / At the top / All over

Hair growth quiz question 3: How much hair have you lost? A lot, it’s obvious to everyone, Some, those close to me notice, A little, only I notice

3. How much hair have you lost? Single-select: A lot, it’s obvious to everyone / Some, those close to me notice / A little, only I notice

Hair growth quiz question 4: When did you start noticing changes to your hair? Over a year ago, In the past year, In the past few months, Not sure

4. When did you start noticing changes to your hair? Single-select: Over a year ago / In the past year / In the past few months / Not sure

Hair growth quiz question 5: With treatment, what results are you hoping for? A stronger, defined hairline, Visibly thicker, fuller hair, More scalp coverage, Keep the hair I have, All of the above

5. With treatment, what results are you hoping for? Multi-select: A stronger, defined hairline / Visibly thicker, fuller hair / More scalp coverage / Keep the hair I have / All of the above

Swap the labels for your products, but keep question 1 about the shopper’s stage and goal. Questions 2 and 3 are your routine signal: a receding hairline points to a different hero than shedding all over. Map each question to its custom field so answers land in ConvertFlow and in your ESP.

Set readable answer values: double-click an answer to open its settings. Each answer has a label, which the shopper sees, and a value, which is stored in the custom field and shown in merge tags and the recap on step 5. Change values to the words you want in the recap, your ESP segments, and your product filters.

3. Connect your ESP on the email step

Step 3 email gate: Almost there, where should we send your plan? First name, email, consent, See My Results button

Go to step 3 (the email gate). Select the form. Actions: connect Klaviyo or your ESP if it isn’t already. Then add automations:

  • Add to list or audience (your quiz or main list)
  • Write quiz answers to custom fields or profile properties
  • Track a quiz-completed event

In the builder: select the form, open the Actions tab, click to add or manage integrations, find your ESP, and click Connect. Then scroll to Automations and click Add Automation, for example “Klaviyo – Add to list”.

Adding a Klaviyo add-to-list automation to a ConvertFlow form

Form in the template: first name, email, email consent, Accept privacy policy and terms of service. Headline: Almost there — where should we send your plan? Button: See My Results.

If this step is skipped, the trial is not activated. Activation is ESP connected plus at least one billable action (list, field, or event). Preview will not fire automations. That is expected. The connection still has to exist.

4. Connect SMS on the SMS step

Step 4 SMS: Get SMS updates, phone number with country code, SMS consent, Subscribe For Texts and No Thanks

Step 4 asks for a phone number with country code and an SMS consent checkbox. Headline: Get SMS updates. Button: Subscribe For Texts. Link: No Thanks. Add an automation to subscribe the phone to your SMS list, and update the consent text with your brand name.

5. Set the results-ready step

Step 5: Your results are ready! with a recap of your hair goal, where you are noticing changes and expected results, and a See My Results button

Step 5 in this template is one screen:

  • Headline: Your results are ready!
  • Personalized line: “{first name}, based on your answers, we’ve built your personalized hair regrowth plan.” Set a fallback first name (for example “friend”)
  • Recap: Your hair goal, Where you’re noticing changes, Expected results
  • Button: See My Results

The recap reads the stored answer values, so fix them on the answers in step 2. Short values (“Hairline”, “Thicker hair”) read better here than the full option labels.

6. Connect Shopify and build the results page

Step 6 results: Your hair regrowth routine with a treatment hero, frequency selector and Start Now button, trust ticks, free gift progress bar, add-ons with Add All To Cart, and FAQs

Connect Shopify under Integrations, then return to step 6. The results page is one scroll:

  • Eyebrow: YOUR PERSONALIZED PLAN. Headline: Your hair regrowth routine. Subhead: Based on your answers, a licensed provider may recommend the following treatment to address your goals
  • Hero: the product hero with the matched treatment (demo: Minoxidil 5% Solution), a frequency selector (one-time purchase or subscription), a Start Now! button, and a “Learn more about this treatment” link. Use conditional recommendations so each pattern gets its hero
  • Trust ticks: Doctor reviewed / Ships free / Cancel anytime (keep only what’s true for your brand)
  • Free gift: “Complete your routine and get a free gift.” Progress bar with the amount left and a Checkout button. Set your threshold
  • Add-ons: the products feed capped at 3, each with a 25% OFF badge and Add to cart, plus Add All To Cart
  • FAQs: How is my treatment plan personalized for me? / What if my treatment isn’t right for me? / Can I retake the quiz for a new plan?

How the product settings work

Choose the product source: select the product hero or products feed, then pick the source. Use your connected Shopify store, or ConvertFlow custom products for a plan or bundle SKU you’ve set up separately.

Choosing the product source and filters for a ConvertFlow products element

Add conditional recommendations per answer: click to add a conditional product recommendation, add conditions based on quiz answers (ALL or ANY must match), then set which products show for that condition. Do this on the hero and on the add-ons feed so both change together. No separate campaigns or steps are needed for each plan.

Or let the quiz pick the products: the Quiz Auto Recommendation filter uses submitted answers and custom fields to filter your catalog automatically. Exclude questions that shouldn’t affect the products, like when they first noticed changes.

Cap the add-ons at 3: in the products feed’s design settings, set the maximum number of products to 3.

Don’t split the hero, gift bar, add-ons, and FAQs into separate steps. Keep it as one personalized sales page (you’ll link back to it from email and SMS).

7. Launch the URL, then preview the full path

Set the landing page domain and path. Save. Preview the whole funnel: intro, questions 1 through 5, email, SMS, results ready, results.

Launching a ConvertFlow quiz funnel as a landing page with a custom domain and path

Confirm:

  • 6 campaign steps
  • First name merges into the results-ready line, with a fallback when empty
  • Recap shows readable values, not stored keys
  • Forms submit (automations won’t run in preview), and No Thanks skips SMS
  • Real products show in the hero and add-ons, not placeholders
  • Different hair loss patterns produce different routines
  • The hero defaults to the subscription option
  • Start Now!, Add All To Cart, and Checkout all work
  • Provider and “Doctor reviewed” claims are true, or removed
  • No thank-you dead end

Then activate. In the dashboard, watch completion, answer mix, attributed orders, and AOV against the benchmark for your AOV band.

In the dashboard: you can see how many visitors progress through each step, which answers they pick, and the orders, revenue, and AOV attributed to each answer.

Then split-test: once you have orders, test subscription-first vs. one-time-first on the hero, the free-gift threshold, and which add-ons make up each routine.

Follow up with personalized email and SMS flows

Most quiz takers won’t start treatment on their first visit, and the ones who do need to stay on it for months. Build flows for both.

8. Build a quiz follow-up flow in your ESP

Trigger it off the quiz: start the flow from the quiz-completed event or the list from step 3. Exit anyone who orders.

Split by answers: use conditional splits on hair goal, area, and expected results. A hairline shopper hears about hairline treatment; an all-over shopper hears about thickness.

  • Email right away: their routine and the link back to their results
  • Email a day later, if they haven’t ordered: how the treatment works and what to expect month by month
  • SMS for shoppers who opted in: a short reminder with the same link
  • For customers: check-ins at 30, 60, and 90 days so they stay on the routine long enough to see results

9. Skip returning visitors straight to their results

Without skip logic, someone who clicks back from email lands on the intro and has to retake the quiz. Skip logic checks each visitor when they reach a funnel step and jumps them to the right one.

Adding skip logic to a funnel step in ConvertFlow

In the builder: click to manage your funnel, edit the intro step, and click + Add Skip Logic. Add the condition that the visitor has already completed the quiz, then under Jump To A Funnel Step choose the results step. Add the same logic to the email and SMS steps so a known subscriber never sees the form twice.

Skip logic settings: if conditions are met, jump to the results funnel step
